Primary Care Physician We are looking for a primary care physician to join our team full-time at Conviva Primary Care. Main Responsibilities Deliver comprehensive outpatient care to adult and senior patients. Maintain accurate medical records and documentation. Diagnose and manage moderately to complex medical conditions. Coordinate referrals and collaborate with specialists. Participate in clinical quality improvement projects. Work with interdisciplinary teams to ensure holistic care. Contribute to projects and innovations in care delivery. Required Qualifications MD or DO from an accredited medical school. Active, unrestricted medical license in the state of practice. Board Certification in Family Medicine, Internal Medicine, or Geriatric Medicine (or board eligible). If selected for this role, you will be required to be screened for TB. Must be willing to participate in on-call coverage by phone. Preferred Qualifications Two years' experience in value-based care or managing high-acuity geriatric patients. Scheduled Weekly Hours: 40 Pay Range: $255,000 - $298,500 per year. This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance. Associates may receive a bonus for the referral of external candidates to this requisition, provided that all other eligibility requirements are met.
