MHM Support Services

Mercy seeks a Community Health Worker to serve as a trusted liaison between health and community services and to help improve access to care. You will document in an EMR, collaborate with teams, and uphold Mercy Service Standards while supporting underserved community members. The role requires a High School diploma, 1–3 years CHW experience, and Mercy-provided CHW training within 6–12 months; proficiency with Microsoft applications and reliable transportation are expected. #J-18808-Ljbffr MHM Support Services
