Under the direction of the Director of Nursing the LPN implements the nursing process and administers skilled professional nursing services, care, and leadership while providing, delegating, evaluating and coordinating comprehensive professional nursing care. This person will be responsible for the monitoring and delivery of health and habilitation supports for individuals, and accepts responsibility for the direction, monitoring, and delegation of activities related to the provision of medication and additional activities by medication aides. Health care will be provided following physician’s orders, and nursing interventions will be documented. The LPN nurse case manager will adhere to Duet policy and procedures, as well as applicable law, and regulations and standards and will also carry out the mission, core values, and quality commitment of Duet.ESSENTIAL JOB FUNCTIONS:Reports to work as scheduled on a regular and reliable basis.Must not appear on the APS/CPS Registry for abuse/neglect, or have been dismissed from prior employment due to suspected abuse/neglect. Must be able to pass a criminal background check.Provides professional nursing care utilizing all elements of the nursing process.Ability to understand and support the community-based philosophy while overseeing and ensuring the health, safety, and well-being of the individuals supported in their homes and/or day programs; therefore being attentive to the persons’ personal choices and interests and interacting appropriately with the individual receiving support.Performs the following functions related to the assessment, planning, implementation and evaluation of individuals on the case management caseload:Assessment- Uses physical assessment skills to provide on-going assessments/observations. Identifies and documents changes in the person’s health status and reports to the RN case manager.Planning- The LPN Case Manager collaborates with the RN Case Manager, the individual, staff, health care professionals and the ISP team to establish an individualized health support plan to achieve optimal health.Implementation- Delegates and monitors to direct care staff and insure that the staff implements interventions based on the established individualized plan of care. Collaborate with other health care professionals and communicate with direct care staff to assure that the needs of the person are being met. Utilizes therapeutic communication techniques that assist the person and the staff to resolve or cope with problems appropriately within their scope of practice.Evaluation- Evaluates the effectiveness of nursing interventions, and participates with the RN Case Manager, the individual, staff, health care professionals, and ISP team members in the evaluation process toward achieving person centered goals.Ability to delegate appropriate nursing care as necessary in accordance with the plan of care and accepts the responsibility and accountability of the delegation to direct support staff.Demonstrate leadership with regard to the individual’s healthcare. Demonstrate leadership and creativity in problem resolution and in proposing new ideas for the individual’s care.Trains direct support staff for appropriate special care procedures, training on medical equipment, cares of the individuals, medication issues, retraining etc. as needed.Ability to use appropriate nursing judgment, schedule and provide home and day program nursing reviews as necessary to meet the needs of the individual. Nursing observations and reviews will be documented and changes in the Medical Status reported to the RN Nurse Case Manager.Ability to keep thorough and legible professional documentation of medical issues while ensuring timely and accurate completion of required documentation. The LPN Case Manager will audit medical documentation.Act as medical liaison and consultant for agency personnel while providing technical and professional assistance related to individual’s health. Delegate responsibility and provide medical training for non-nursing staff.Communicate with physicians, vendors, social workers, etc. as needed to to provide quality care for the individuals. This task may be delegated as appropriate.Facilitate communication of information between hospital staff and direct support staff during hospitalizations.Answer administration and direct support staff questions pertaining to the individual during office hours Monday-Friday.When requested, performs all essential responsibilities of the LPN while providing direct nursing support at the Medical Support Units and the Jerry Graesser Center (JGC).During normal work hours accept reassignment to provide direct support at MSU, JGC, etc. as needed for situations when unexpected staffing needs arise, emergency response is needed, or if additional staffing is required.Ability to attend hospital discharge meetings, in-services, educational programs, and essential agency training.Participate in required ISP meetings on nursing caseload and covers other nursing caseloads as necessary.Ability to work effectively with coworkers.Communicate with coworkers both verbally and in writing.Recognize and respond appropriately to nonverbal and situational cues from coworkers, such as when there is a need for physical assistance with the care of an individual or verbal reinforcement.Create a positive work environment in the home/day program and be a motivator and leader for staff.Serve as a role model for staff in tone, morale, and professionalism in interactions with the people supported, other Duet employees, family members and friends, DHHS staff, day program personnel, medical personnel, vendors and others.Ensure that staff documentation and follow-through meets requirements with regard to med sheets, medical charting, seizure records, medication history, hospitalization records and other areas of documentation.Give staff feedback on job performance in a prompt, tactful, direct and consistent way; follow through with the Residential Supervisor or Coordinator with any concerns.Be attentive and responsive to employee concerns and refer issues to appropriate personnel in the company as needed. Demonstrate tact, kindness and concern toward staff.Promotes harmonious relationships and favorable attitude among staff.Effectively works with legal representatives, service coordinators, and team members.Act rapidly and effectively to manage self, individuals supported, and other employees during emergency situations.Assume medical on-call rotation responsibility evenings and weekends.Provide Flu Vaccinations to agency employees.Follow HIPPA guidelines to ensure confidentiality.Adhere to all company policies as stated in the current Employee Handbook, the ENHSA Human Resources Manual and the Duet Policy and Procedures.OTHER JOB DUTIES:Other duties as assigned.Assists in the orientation and training of new employees. Teach Medication Aide, Medication Renewal, and other medical training classes as assigned for Duet staff.Must be able to Jump-start a vehicle and perform basic vehicle maintenance as required by the Daily Vehicle Checklist.EDUCATION, TRAINING AND EXPERIENCE:Graduate of an accredited School of Nursing and State of Nebraska LPN or current LPN licensure from a state participating in the “multistate privilege to practice” compact with Nebraska required.Minimum two years’ experience as a LPN with experience within the two years prior to date of application required.ESSENTIAL KNOWLEDGE, SKILLS AND ABILITIES:Knowledge of, and ability to apply, professional nursing theory, quality standards, procedures, protocols, practices and ethics.Ability to support and relate comfortably, confidently, and effectively with individuals with developmental disabilities.Possess judgment and decision-making skills to perform the functions of the job day to day and in crisis situations.Ability to work alone or in a team situation with other professionals in various disciplines to pinpoint problems, to implement effective actions, and to motivate change to individuals served.Ability to adherence to values and ethics commonly associated with professionals having access to confidential and sensitive person-centered information.Ability to serve all individuals without discrimination as to race, color, creed, age, sex, sexual orientation, or abilities.Knowledge and sensitivity to cultural and socioeconomic differences.Ability to analyze, plan, implement, and evaluate individual plans of care and treatment.Knowledge of current medications, their actions, interactions, uses and side effects.Knowledge of clinical instruments, equipment and procedures.Knowledge of nursing interventions and methods.Ability to analyze and evaluate proper care and treatment.Ability to communicate clearly, both orally and in writing; and demonstrate skill in preparing and maintaining records.Ability to learn specialized care applicable to work assignment.Ability to provide training to staff.Knowledge of, and ability to apply, clinical therapeutic models of individual care with emphasis on interdisciplinary team approach.Ability to listen and provide empathetic assistance to individuals served and families in meeting their needs and carrying out prescribed treatments.Demonstrate good nursing judgment and excellent observational, interpersonal and oral and written communications skills.Ability to act as a role model to health care staff by demonstrating and teaching professional behaviors.Ability to encourage and facilitate cooperation, trust, and group identity to foster commitment and teamwork. A pre-employment physical and backscreen will be completed to qualify you for the essential functions of the job. Must be able to lift at least 70 pounds, to assist supported individuals in everyday tasks that require mobility they may be limited to (i.e. assisting in/out of wheelchair, in/out of vehicles, Hoyer lift, raised toilet seats, etc.). Must be able to bend, stoop, kneel, squat, walk, sit, stand, climb, push, carry, and grasp. Work is normally performed in the Duet office, Duet Residential and Day Service settings, MSU, and the JGC. Work environment involves exposure to potentially dangerous materials and situations that require adherence to safety precautions and may include the use of protective equipment.
