Role Description The Nurse Advocate/Care Coordinator, PRN will provide direct patient advocacy and care coordination services to individuals navigating healthcare in Dallas, TX. In this PRN, remote and on-site role, the nurse advocate will meet with patients and families to review medical histories, clarify diagnoses and treatment plans, and help them understand provider recommendations. Daily responsibilities include communicating with healthcare teams, scheduling and coordinating appointments, reviewing medical records, and helping patients prepare for visits and procedures, and attending appointments with patients. This job requires a lot of "following up."

The role also involves educating families on caregiving strategies, documenting interactions and care plans, and escalating concerns firmly, but professionally, to providers when needed to support safe, high-quality care. The Nurse Advocate/Care Coordinator will collaborate closely with internal team members and proactively seek out community resources to support patient-centered outcomes.

Qualifications

• Clinical expertise in nursing practice, including strong assessment skills, knowledge of common diagnoses and treatments, and ability to interpret medical records.
• Care coordination and case management skills, such as organizing appointments, tracking care plans, and facilitating communication among multiple providers.
• Patient advocacy and communication skills, including active listening, clear explanation of complex information, and respectful representation of patient preferences.
• Strong organizational and time-management skills to handle multiple patients, changing priorities, and detailed documentation.
• Ability to build trust and rapport with diverse patients and families, demonstrating empathy, cultural sensitivity, and nonjudgmental support.
• Proficiency with electronic health records and basic office technologies to document care, communicate with providers, and maintain accurate records.
• Current Registered Nurse (RN) license in the state of Texas and good standing with the applicable licensing board.
• Prior experience in care coordination, case management, patient advocacy, or a related nursing specialty is strongly preferred. Spanish proficiency a huge plus.
• Safe and reliable transportation, which may be used to transport patients.
• Must be available during business hours, on agreed upon schedule.
• Reliable internet connection and smart phone

Preferences:

Preferably located in a central location, Irving, Dallas, Arlington.

At least 1+ year of hospital experience, LVN okay.

Oncology and case manager experience bonus, would prefer our clinical experiences complement each other.

Salary begins at $40+ but is commiserate with experience.

Hours vary week to week, but you will be able to look for your own clients under my umbrella as well.
