Monitor residents’ vital signs, assess changes in condition, and report findings to the supervising nurse. Perform basic wound care, dressing changes, and support activities of daily living as needed. Maintain patient safety with fall precautions, infection control, and adherence to care plans. Collaborate with the RN and interdisciplinary team to coordinate patient care and communicate changes. Document nursing actions accurately in the electronic chart and ensure care plans are followed.
