Description This position serves as a Medically Assisted Treatment Case Manager/Coordinator for the Substance Use Program. Responsibilities involve program oversight, administrative support, and acting as the point of contact with the Department of Public Health for Suboxone, Naloxone, and Vivitrol treatment services across CHC. The role requires collection and entry of all data into appropriate systems, coordination of follow-up appointments, and documentation. It works in conjunction with the CMO, the Behavioral Health Director, and the COO, requiring excellent decision-making, problem-solving, and attention to detail. The role involves handling highly sensitive medical and behavioral health information, requiring a clear understanding of confidentiality. Minimum Qualifications 2‑year college degree in hospital, public or business administration, health or human services or related disciplines; 4‑year degree preferred. Associate’s degree is acceptable if licensed as an RN in Massachusetts. 12–24 months of case management experience along the care continuum. AHA BLS Health Care Provider certification or equivalent. Strong communication skills evidenced by preparation of oral and written materials, presentations, reports, memoranda and correspondence. Analytical ability demonstrated by developing solutions and recommendations with guidance from superiors.
