The Care Navigation Team Leader supervises the Care Navigation team and supports the implementation and growth of the Care Navigation Program while ensuring high quality patient and referral source experiences.

What you will do:

  • Supervise Care Navigation staff including: Registered Nurses (RNs), Licensed Practical Nurses (LPNs), Care Navigation Social Workers, Care Navigation Support Agents
  • Carry out additional supervisory assignments as directed by the Director of Navigation Services.
  • Participate in the planning and implementation of the Care Navigation Team Development Project.
  • Assist with onboarding and training of new Care Navigation staff.
  • Provide supervisory support for evening triage as needed.
  • Participate in quality, performance, and process improvement initiatives.
  • Cross-train to provide coverage for additional Care Solutions Center leadership roles including: On-Call Teams, Admissions/Liaison, Triage

Success capabilities:

  • Registered Nurse (RN) in the state of Ohio strongly preferred
  • Minimum of two years' hospice experience required
  • Strong communication, leadership, and managerial skills
  • Ability to lead a large, multidisciplinary team, including managing performance and disciplinary action when necessary
  • Strong technology proficiency
  • Proven ability to manage multiple priorities and operational demands

Physical requirements:

This is sedentary work requiring the exertion of up to ten (10) pounds of force occasionally and some light work and lifting requiring the exertion of up to 20 pounds occasionally; work requires sitting, stooping, bending, kneeling, reaching, standing, fingering, grasping and repetitive motions; vocal communication; hearing, and visual acuity. Employee is not subject to adverse environmental conditions. This position has the potential to be exposed to environmental elements, internal and external, such as infectious disease, bloodborne pathogens, and chemical and/or chemical fumes, (pet dander and/or exposure to allergens) and adverse effects of inclement weather that may occur as well as potentially difficult patient and family situations. The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
