Home Health Registered Nurse - Home Health

Schedule
Shift: Day 5x8-Hour (08:00 - 16:30)

Assignment Details
Contract Length: 13 weeks
Guaranteed Hours: 40

Requirements
- Must have both Oregon and Washington state licensure
- 1 year of skilled home health experience required
- BLS certification required
- OASIS experience required

Roles & Responsibilities
- Provide direct patient care in home settings.
- Conduct patient assessments and develop individualized care plans.
- Educate patients and families on health management and self-care techniques.
- Maintain accurate and timely documentation of patient care and progress.
- Collaborate with interdisciplinary teams to optimize patient outcomes.
- Comply with facility policies and procedures.

Compliance / Notes
- Traveler is expected to have their own transportation.
- Facility requires primary series COVID vaccination in addition to one booster.
- Dress code: Business casual; scrubs are allowed but not required.
- Orientation hours: 40 hours.
