Registered Nurse - Home Health

In-Home Nursing Assessments And Care Coordination

Provide in-home nursing assessments and care coordination for patients enrolled in managed care programs. The role requires conducting hybrid in-field and telehealth evaluations to ensure quality care and compliance with state and agency regulations.

Responsibilities:

• Conduct initial in-home pre-evaluations assessing patient health status, strengths, care needs, and preferences.
• Review medical documentation and health referral forms relevant to each case.
• Record evaluation data accurately into electronic systems and transmit as required.
• Document concerns, conflicting information, or issues identified during assessments.
• Collaborate with consumers and healthcare teams to develop comprehensive care plans.
• Communicate with the Call Center and eligible consumers to facilitate enrollment into managed care health plans.

Qualifications:

• Valid RN licensure in New York State.
• Minimum of two years’ recent experience in a home health setting within the last 15 years.
• Ability to perform comprehensive assessments and utilize technology effectively.
• Strong organizational, interpersonal, and communication skills.
• Demonstrated sensitivity and effectiveness in working with diverse populations.
• Experience working independently in a fast-paced environment.
