Hospice Intake Coordinator: Admissions & Family Liaison

Serenity Hospice Care is seeking an organized and compassionate Intake Coordinator to join our team in New Jersey. The role involves working with hospice patients and families in a hospital setting, serving as a liaison between facilities, marketing, social workers and nurses. Hours are Monday–Friday 8:30AM–4:30PM. Responsibilities include processing referrals, verifying insurance, tracking census and care levels, coordinating with staff to place patients, and ensuring accurate documentation for #J-18808-Ljbffr Ocean Healthcare
