Your role at BHSN:

This position provides fiscal, programmatic and operational oversight of the Breakthrough II Apartment Treatment Program, a 45-bed OMH Licensed housing program that assists adults living with serious mental illness develop the skills to transition to independent living. Supervisor works collaboratively with BHSN team members/departments including but not limited to Director, Nursing, Crisis, Finance, Clinic and Facilities Department to help lead and support the team and those we serve.

Work Schedule: Monday - Friday, day shift, will require flexibility to meet needs of the program

Essential duties & responsibilities:

• Oversees the daily operations of the Apartment Program to ensure resident/program needs are met including but not limited to:

• Facilitates, develops and assists with mentoring, on-boarding, training, supervising and monitoring Apartment Program staff and interns

• Establishes and coordinates staff schedule to ensure adequate coverage is maintained to ensure resident needs are met

• Collaborate and communicate effectively with team members to ensure resident needs are met including but not limited to coordination of mental health/substance use/primary care services, obtaining funding, transportation, linkage to other services, care coordination and discharge planning

• Coordinates and communicates all aspects of admissions and discharges to the team

• Possesses a working knowledge of and follows 14 CRR NY-XIII 593 and 595 Office of Mental Health regulations and BHSN policies/procedures as it relates to residential programs

• Participate in internal and/or external committees as needed to inform and/or screen potential Breakthrough II OMH licensed housing applicants for eligibility and preference for admission

• Works collaboratively with Residential Funding team, Finance Department and outside agencies to ensure resident funding/insurance is in place prior to or upon admission and throughout program stay to obtain/maximize program fee and Medicaid revenue; oversees program finances to ensure fiscal responsibility and that resident monies are received and documented accordingly

• Direct oversight of Breakthrough II Administrative Assistant(s), Breakthrough II Peer Engagement Specialist(s), Breakthrough II Crisis Intervention Specialist, Senior Apartment Program Case Manager, Apartment Program Case Managers, and interns.

• Works collaboratively with internal and external entities to coordinate, communicate and reinforce integrated residential and medical treatment to ensure client coordination of care is occurring

• Ensures and monitors that monthly site safety checks and resident fire drills are completed correctly and any deficiencies resolved promptly to ensure a safe and clean living environment

• Collaborates with participating landlords to resolve tenant/landlord disputes, maintain safe environments, review and negotiate lease renewals

• Collects and organizes various data throughout the year as requested (i.e. client satisfaction surveys, occupancy rates, annual reports, etc )

• Participates, facilitates and ensures accurate documentation of all staff meetings

• Collaborate and coordinate with Adult Residential Nursing team to insure the implementation, integrity and monitoring of medical management program for the Apartment Program is occurring

• Ensures that regularly scheduled treatment team meetings are scheduled and Apartment Program team members are participating and effectively communicating outcomes to team members to ensure resident’s needs are met; additionally may coordinate and participate in treatment team meetings

• Provides, monitors and ensures that all participants receive restorative services as indicated on their service plan and accurate documentation of services in a timely manner in accordance with Federal/State regulations and BHSN policies/procedures

• Monitor and report outcomes/Quality Management activity

• Observe the resident self-administration of medication according to established protocols

• Works with Adult Residential Nursing Team to m aintain accurate medication records, storage, handling and monitoring of medications

• Assist residents in maintaining safe and sanitary dwelling and p romptly report facility/apartment maintenance issues as appropriate to insure a safe living environment for tenants

• Adjusts schedule to meet the needs of the program and ensure support services are provided

• Complies with all BHSN and regulatory policies and procedures

• Collaborate with Compliance Department to ensure timely and accurate reporting of incidents and to preserve the integrity of the program in accordance with State, federal and company policies and procedures

• Provides and accurately documents direct support services with individuals in accordance with individualized service plan and in accordance with Federal/State regulations and agency policies/procedures

• Review and sign off as qualified mental health staff on all service plans developed by staff to ensure they meet regulations and they promote resident independence, choice and outcomes

• Attend various meetings with internal and external community partners

• Additional duties as assigned within the scope of this job description

Your skills and qualifications:

• Master’s and/or Bachelor’s degree in human services related field OR Associate’s degree in human services related field AND 3 years’ experience in human services OR High School Diploma AND 5 years’ experience in human services required

• Case Management and/or care coordination experience required

• Experience with crisis resolution and strong de-escalations skills required

• Strong interpersonal, communication, time management, problem solving, leadership, supervisory and organizational skills required

• Supervisory Management experience required

• 3 years’ experience with vulnerable population required; experience working with individuals recovering from severe mental illness and/or substance use disorders strongly preferred

• Proficient in Microsoft Office software such as MS Word & Excel are required

• Reliable transportation to transition between varying program sites required

• Must be able to lift up to 25 lbs and have the ability to climb and descend stairs

• Ability to work a flexible schedule to accommodate the needs of the program, including non-traditional business hours

• Participate in lifting and moving resident’s belongings

• Valid NYS driver’s license, acceptable to BHSN insurance carrier, for required local travel

• CPR and NARCAN certification required within 60 days of employment
