Matrix Medical Network seeks a Nurse Practitioner to perform in-home Comprehensive Health Assessments, reviewing health history, medications and social factors to identify care gaps. Visits occur at members’ homes or SNFs, with collaboration from physicians and care teams. Applicants must hold NP licensure, board certification, and be able to travel extensively.
