Registered Nurse (RN) - Home Care

Location: Saco, Maine

Assignment Duration: This is a 13-week contract position.

Schedule: Shift: 8:00 AM - 5:00 PM; Every other weekend rotation with a day off during the week or permanent schedule options based on agency needs.

Job Description: We are seeking a dedicated Registered Nurse (RN) to join our home care team in Saco, Maine. This role requires a strong commitment to providing high-quality patient care in a home setting, with expectations for managing a caseload of 20-25 patients and conducting approximately 6 visits per day.

Responsibilities:

• Manage patient care, including assessments, evaluations, and adherence to care plans.
• Utilize OASIS documentation for patient evaluations and care planning.
• Perform clinical tasks such as IV therapy, tube feeds, tracheotomy care, and wound care.
• Engage in collaborative practice with healthcare teams to optimize patient outcomes.
• Respond to on-call requirements as needed, with calls typically occurring 2-4 times per month.

Required Experience / Certifications / Licensure:

• Minimum of 2 years of nursing experience in home care or related fields.
• Current RN license in the state of Maine.
• BLS certification is required.
• Experience with Epic and OASIS documentation is preferred.
• Skills in IV therapy, tube feeds, tracheostomy, and wound care are a plus.
