Job Title: Case Manager Nurse Profession: Registered Nurse Specialty: Case Management Duration: 13 Weeks Shift: Day Hours per Shift: 8 Hours (08:00 - 16:30) Experience: Minimum of 2 years of acute care case management/discharge planning experience License: CA state license required; pending license accepted but must be licensed upon start date Certifications: Relevant certifications preferred Must-Have: - InterQual experience preferred - Experience in acute hospital settings - Competent in care coordination and discharge planning - Knowledgeable in Medicare guidelines - Ability to perform independently Description: This position is a full-time Case Manager Nurse role requiring expertise in managing patient care. The Case Manager will coordinate care processes for patients to ensure effective discharge planning. The role may require coverage on weekends and holidays, with expectations to be available for every other weekend. The nurse will work with interdisciplinary teams, including physical therapy, social services, and rapid response teams. Strong skills in utilizing assessment tools and guidelines for determining medical necessity are essential. Experience with diverse patient populations, including behavioral health, is beneficial. This role involves participation in family meetings and the development of treatment plans. Respond to patient needs while adhering to established protocols and regulatory requirements. Strong organizational skills are necessary to manage a caseload effectively. Ability to arrange for necessary medical equipment and transportation services. Candidates must possess the ability to navigate complex healthcare systems. Prior experience in a case management role focused on acute care environments is required.
