Humana Inc. is seeking a Care Coordinator to support Illinois Medicaid Members in the Humana Gold Plus Integrated program. You will assess needs, develop personalized care plans, and connect members with resources to improve health and well-being. You will report to the Manager, Care Management and may support MLTSS or LTSS as needed. This field role requires 75-90% travel to visit members, work Mon-Fri 8am-5pm CST, and compliance with Humana safety programs. #J-18808-Ljbffr Humana Inc
