At Phoenix Hospice, our RN Case Managers walk alongside patients and families during one of life's most vulnerable seasons. You'll provide skilled, compassionate care while helping families navigate difficult decisions with clarity, dignity, and heart. Every visit matters. Every conversation counts.

You won't do this work alone. You'll be supported by a strong interdisciplinary hospice team that believes in teamwork, purpose, and caring for the caregiver just as much as the patient.

Active Missouri or Compact RN license

Hospice or Long-Term Care nursing experience preferred

Strong assessment, communication, and care coordination skills

Valid driver's license and auto liability insurance
