Seven Healthcare are seeking a dedicated RN – Home Health – Travel for assignment in York, PA. This role focuses on providing hospice care with responsibilities including admissions, routine visits, and health assessments. As part of a supportive and professional environment, the registered nurse will actively engage in health maintenance, disease prevention, and case management for individuals in their home setting.

Key responsibilities of the RN – Home Health – Travel include:

• Observing, assessing, planning, implementing, and evaluating nursing support for well, chronic, or acutely ill individuals
• Working closely with other health professionals/providers to address the health needs of persons served
• Coordinating communications and actions of various disciplines related to health and safety supports
• Managing the ordering, reviewing, monitoring, and disposal of medications as prescribed
• Ensuring policies on medication administration and storage are followed and reviewed annually
• Communicating progress reports to the physician when client conditions change
• Completing nursing documentation as required
• Supervising licensed practical/vocational nurses
• Scheduling health-related appointments and ensuring timely implementation of findings
• Providing training for new employees and annual recertification training on health and safety topics

Requirements of the RN – Home Health – Travel include:

• PA RN License
• Valid Driver's License
• Valid Car Insurance
• Valid BLS CPR
• Hospice and HCHB experience required
