Case Manager & Prevention & Education Coordinator
Alfred University
Reporting to the Assistant Dean of Students for Residential Communities & Student Well-Being, this position serves as a key member of the Student Experience team by connecting students with campus and community resources, coordinating individualized support plans, and developing evidence-informed educational programs focused on health promotion, mental health awareness, substance misuse prevention, interpersonal violence prevention, and student wellness.

Rate: $22.20-$23.54, 35hrs/week 10-month position Essential Responsibilities:Student Case Management (40%)
- Serve as a central point of contact for students experiencing personal, academic, behavioral, health, or wellness concerns.
- Assess student needs and coordinate individualized support plans in collaboration with campus partners.
- Conduct outreach to students referred through faculty, staff, residence life, Public Safety, CARE Team, Student Conduct, and other reporting mechanisms.
- Monitor student progress through regular follow-up meetings and communication.
- Maintain accurate documentation utilizing the university's student success and case management systems.
- Coordinate referrals to campus resources including Counseling Services, Health Services, Academic Success, Accessibility Services, Financial Aid, Student Conduct, Residential Communities, Title IX, and community agencies.
- Participate in the University's CARE/Behavioral Intervention Team and assist with coordinated response efforts.
- Support students returning from medical or mental health leaves, hospitalization, or other significant interruptions.
- Develop and implement a comprehensive prevention education strategy aligned with national best practices and institutional priorities.
- Coordinate educational programming focused on:
- Mental health awareness, suicide prevention, alcohol and other drug education, sexual violence prevention, healthy relationships and consent, bystander intervention, stress management and resilience.
- Partner with Residential Communities to integrate wellness education throughout the residential experience.
- Coordinate campus awareness campaigns during national health observances and prevention months.
- Collaborate with student organizations, Athletics, Residential Communities, Title IX, Student Engagement, and Orientation to deliver educational initiatives.
- Evaluate program effectiveness through assessment and learning outcomes.
- Build meaningful relationships with students through visible outreach and engagement.
- Assist with orientation, Welcome Week, residence hall programming, and campus wellness initiatives.
- Support peer educator and student ambassador programs related to health and wellness.
- Represent Student Experience at campus events and collaborative initiatives.
- Collect and analyze data related to case management trends, outreach efforts, and prevention programming.
- Prepare annual reports and assessment summaries demonstrating student impact.
- Assist in policy development and campus-wide wellness initiatives.
- Remain current on emerging issues and best practices in higher education, case management, and prevention education.
- Serve on university committees as assigned.
- Required:
- Master's degree in Higher Education, Student Affairs, Counseling, Social Work, Public Health, Health Education, Psychology, or related field.
- Two years of professional experience in higher education, student affairs, counseling, health promotion, case management, or related field.
- Demonstrated knowledge of student development theory and wellness promotion.
- Strong interpersonal, crisis response, and organizational skills.
- Ability to interface with distressed students, maintain confidentiality, function independently, use discretion, exercise judgment to make decisions, and be able to re-prioritize on an ongoing basis.
- Demonstrated ability to work with persons of diverse social, cultural, economic, and international backgrounds in support of student services, wellbeing and learning.
- Ability to manage sensitive information with professionalism and confidentiality.
- Excellent written and verbal communication skills.
- Preferred:
- LMSW with 2+ years of social work experience
- Experience with student conduct, behavioral intervention teams, CARE teams, or case management.
- Experience developing prevention education and health promotion programming.
- Knowledge of trauma-informed practices and motivational interviewing.
- Familiarity with student success platforms such as Navigate360, Maxient, Medicat, or similar systems.
- QPR, Mental Health First Aid, or other relevant certifications.
