Job Title: Physical Therapist (Home Health) Location: Fayetteville, NC 2830 (Cumberland, Hoke, and Robeson Counties) Duration: 3 months contract with possible extension Schedule: Monday – Friday, 8:00 AM – 4:30 PM Weekly Guaranteed Hours: 40 H On-Call Requirement: None Mileage Reimbursement: None Bonus Position: n Bonus Position Summary Seeking a dedicated and experienced Travel Physical Therapist (PT) to provide high-quality, patient-centered care in a home health setting. The Physical Therapist will be responsible for evaluating patients, developing individualized treatment plans, and delivering therapeutic interventions to improve mobility, function, and overall quality of life. Key Responsibilities Evaluate and assess patients’ physical condition, mobility, strength, and functional abilities. Develop and implement individualized treatment plans to improve patient outcomes. Deliver skilled physical therapy interventions in accordance with physician orders and care plan. Monitor patient progress and modify treatment plans as needed. Educate patients and families on exercises, safety, and rehabilitation techniques. Maintain accurate and timely clinical documentation in accordance with agency and regulatory standards. Collaborate with physicians, nurses, and interdisciplinary care teams to ensure coordinated patient care. Ensure compliance with Medicare guidelines and home health regulations. Participate in agency quality improvement initiatives and maintain patient safety standards. Required Qualifications Current, active Physical Therapist (PT) License (state-specific as required). Valid driver’s license and ability to travel within assigned coverage areas. Experience in home health or outpatient physical therapy setting. Strong clinical assessment and patient care skills. Preferred Qualifications Experience using HomeCare HomeBase (HCHB). Prior home health experience. Strong time management and ability to work independently.
