We are currently seeking an Occupational Therapist (OT) to join our in-house rehabilitation team. This role is ideal for a compassionate, experienced therapist looking to make a meaningful impact with a flexible schedule. Key Responsibilities
- Address patient needs related to daily living and functional independence
- Develop individualized treatment plans using evidence-based interventions
- Deliver occupational therapy services to enhance safety, mobility, and performance in ADLs and IADLs
- Teach patients adaptive strategies and task modifications to improve engagement
- Recommend and train patients in the use of adaptive equipment
- Educate family members and caregivers on home care strategies
- Document patient assessments, treatment plans, progress notes, and discharge summaries
- Maintain communication with interdisciplinary team members to ensure coordinated care
- Follow up on patient progress and adjust interventions as needed
- Bachelor's degree or higher in Occupational Therapy from an accredited program
- Current license to practice Occupational Therapy in the state of Pennsylvania
- Strong clinical knowledge of therapeutic principles and treatment methods
- Familiarity with ADL evaluation tools and adaptive equipment
- Excellent interpersonal and communication skills
- Ability to work independently and as part of a collaborative clinical team
- Prior experience in a skilled nursing or long-term care setting preferred
- In-house therapy team with a focus on clinical excellence
