Physical Therapist – Skilled NursingSchedule: - Shift: DaysAssignment Details: - Guaranteed Hours: 40 - Contract Weeks: 13Requirements: - Active license as a Physical Therapist in the applicable state - Bachelor's degree required; Master's or Doctorate preferred - Basic Life Support (BLS) certification preferred - Skilled Nursing experience preferredRoles & Responsibilities: - Evaluate residents and establish therapeutic plans of care to enhance functional abilities and independence in daily living activities. - Document all evaluative and therapeutic interactions and ensure documentation is accessible and understandable to the care team. - Collaborate with physicians and healthcare team members regarding discharge needs and residents' plans of care. - Modify treatment plans as necessary and monitor treatment progress toward established goals. - Provide education to residents and caregivers regarding therapies and appropriate assistive devices. - Supervise assistant-level staff and ensure adherence to regulatory standards regarding their use. - Participate in continuous education and development of departmental programs and services.
