CLINICAL PHARMACIST

Clinical Pharmacist
The Clinical Pharmacist will provide clinical pharmacy services for ambulatory CCHS patients in collaboration with health center staff providers.
Duties and Responsibilities
The Clinical Pharmacist will:
- Provide care for outpatients within their scope of training in collaboration with CCHS providers. Care will consist of assessment, ordering and interpretation of data, development and implementation of a therapeutic plan including prescription writing, and procedures within their capacity of expertise, protocols, and available equipment and appropriate follow-up. The Clinical Pharmacist will thoroughly, accurately, and promptly document all work completed in the practice management system.
- Partner with the PCMH team (PCPs, Behavioral Health Consultant, Spiritual Health Provider and Nurses and support staff) at their assigned clinic to improve the health of the patients, identify high risk patients in the daily evening huddle that require detailed review by PCP or separate visit with the Clinical Pharmacist and help to maintain the overall cohesion of the clinic PCMH team.
- Complete one on one clinical visits with patients that have been assigned to them on their schedule by Site Lead Physicians or Site Managers.
- Organize and teach any needed classes as assigned by Chair of Medicine or Pharmacy or Chief Clinical Officer.
- Attend staff and provider meetings and maintain a level of involvement with the health center committees and pharmacy department as needed to ensure quality of care of patients.
- Participate in peer review, performance evaluations, improvement data, and other activities to assess and advance quality improvement measures.
- Meet productivity expectations.
- Collaborate with CCHS Health Center staff physicians and pharmacists to achieve quality care. The Clinical Pharmacist may be asked to assist in the educational training of students.
- Participate in Continuing Education, including Life Support Training, etc. to maintain skills necessary to perform their duties and as required by their boards and the state.
- Sustain the health necessary to perform their duties and maintain current immunization and PPD status.
- Notify CCHS about any problems that occur regarding Drugs, Felonies, Discipline, Loss of license, or other serious issues.
- Perform other duties as assigned by the Chair of Medicine.
- Regularly check mail, e-mail, and voicemail and respond promptly and professionally to all inquiries.
Position Requirements
Education: Doctor of Pharmacy
Experience: Post Degree Residency or Board Certified in Ambulatory Care Pharmacy
Licenses or Certifications: TN License, BLS

Level 1– Requires some concentration and normal attention. Generally, once the job is learned, the tasks can be performed more or less automatically.
Level 2– Requires high periods of concentration intermittently and normal attention. Generally, even once the job is learned, tasks will require normal attention to deal with recurring variables.
Level 3– Requires a high level of concentration and high level of attention intermittently. Generally, the approach to tasks may be consistent, but the number of steps required and/or the number of variables involved creates the possibility of errors unless the incumbent pays close attention.
