Collaborate with interdisciplinary team to optimize patient outcomes. Document assessments, treatments, and outcomes accurately in EMR systems. Experience / Qualifications Licensed Physical Therapist Assistant in good standing; SNF setting preferred. Provide skilled PTA services under supervision in SNF facilities. Develop and implement patient care plans with measurable functional goals. Accurate documentation of assessments, treatments, and outcomes in EMR systems. Identify and report safety concerns and potential risks promptly. Monitor progress; adjust treatments based on objective outcomes and data.
