Requirements

• Active Physical Therapist (PT) license in OH

• Minimum 1 year of physical therapy experience required (home health experience a plus)

• Minimum 1 year of management experience preferred (home health experience a plus)

• Current CPR certification required

• Ability to work independently in a home setting

• Strong communication and patient care skills

• Valid driver's license and reliable transportation required

• Ability to pass background screening requirements

Responsibilities

• Provide physical therapy services in patient homes per plan of care

• Perform patient evaluations and develop individualized treatment plans

• Implement therapeutic exercises and mobility training

• Educate patients and caregivers on treatment plans and progress

• Monitor patient condition and adjust care as needed

• Maintain accurate and timely documentation

• Collaborate with interdisciplinary care team
