Targeted Case Manager

Targeted Case Manager – Integrated Health Care Clinic Liberty Resources is seeking a Targeted Case Manager to work in our Integrated Health Care Clinic in Fulton, NY. The role focuses on providing care coordination and resource linkage for clients served by the clinic, primarily supporting those living with severe mental illness and/or substance use concerns. Position Summary The Targeted Case Manager (TCM) is responsible for coordinating referrals, conducting outreach, and managing the comprehensive care of individual clients to improve overall well-being. Responsibilities Determine recipients’ immediate and ongoing needs for Care Management services, and complete enrollment and referral processes. Conduct ongoing assessments to identify strengths, barriers, and establish clear goals that influence overall health and wellness, linking individuals with additional support services. Coordinate and collaborate with all providers and support services for each client, including physicians, specialists, mental‑health providers, hospitals, and other community resources. Develop a comprehensive Plan of Care in collaboration with the treatment team, ensuring it is shared with all members and that goals are measurable, cost‑conscious, and responsive to individual needs. Maintain relationships with community service providers and ensure continuous support for clients. Establish clear communication lines to ensure high‑quality care delivery. Complete all required documentation on time and in accordance with program standards. Support other community‑based programming and perform additional duties as assigned. Qualifications Bachelor’s degree (minimum) in Psychology, Social Work, Human Services, or a related field. 1–2 years of experience in Behavioral Health, Substance Use, or Social Services. Experience with mental‑health and substance‑use disorders, and with vulnerable populations is preferred. Knowledge of community‑based behavioral health care and case management is an asset. Valid NYS driver’s license and reliable transportation. Salary $50,000 to $55,000 annually.
