Location: Banning Full-Time/Part-Time: Per Diem Shift: -not applicable- Exempt/Non-Exempt: Non-Exempt Pay: $50.00 - $140.00 per visit Job Type: W-2 Per Diem Medical Specialty: Home Health Work Location: On the road We are looking for a clinically experienced nurse who has nursing documentation and charting skills, is a fast learner, and has strong communication skills. Your role includes administering at‑home IVs, changing dressings, cleaning wounds, performing in‑depth assessments, overseeing patient treatment plans, and updating doctors and the agency. Documentation and communication with the agency are done through Synergy Software. Availability Our patients need care 7 days a week. You must be able to work at least two‑three days each week (or ten productivity units), one weekend a month, and two holidays per year (one winter and one summer). Responsibilities Provide nurse visits to home health patients. Perform an initial, comprehensive assessment and upload documentation to the Agency Portal. Obtain and document physician orders. Initiate and regularly re‑evaluate and revise the plan of care in consultation with the assigned Clinical Team Manager. Coordinate care with the Clinical Team Manager, physician, and other members of the home care team, informing them of significant changes in the patient’s condition and needs. Perform re‑certifications, resumption of care, transfers, and discharges as requested by the assigned Clinical Team Manager, completing and submitting all related documentation. Assess the need for services of other team members (PT, OT, ST, MSW, and HHA). Perform Home Health Aide supervisory visits per regulatory requirements. Demonstrate knowledge of regulatory requirements.
