Social Worker, BSW or MSW

Office: Wilson and Greenville, North Carolina Coverage to include: Wilson, Nash, Edgecombe, Beaufort and Pitt County Summary The Social Worker, BSW is responsible for meeting the social needs of patients and their families. The BSW is also responsible for providing emotional support during end of life. The BSW is directly supervised by a MSW. Essential Functions Abides by and supports 3HC's Compliance Program and Code of Ethics, upholding ethical and legal standards. Creates positive experiences for internal and external customers, displaying courtesy, tact, and knowledge of services. Manages assigned cases to achieve positive patient outcomes: provides social work care per physician's plan, consults with physicians on plan alterations, documents changes, liaises between the Hospice program and community agencies for bereavement services, involves patient/family in care planning, evaluates response to interventions, identifies obstacles to compliance, assesses long‑term care needs, and communicates psychosocial information to inpatient facilities. Conducts comprehensive patient assessment to identify necessary care: evaluates caregiver function, social needs, environmental resources, risks of suicide or neglect, cultural diversity considerations, family dynamics, and support systems; assists physicians in recognizing social/mental stressors; provides ongoing bereavement care, short‑term counseling, crisis intervention, advance directive preparation, funeral planning, and financial/legal guidance. Develops and maintains relationships with community resources and facilitates referrals, ensuring patient/family access when home health care is unsuitable. Evaluates caregivers for high‑risk bereavement and completes a face‑to‑face bereavement assessment within seven days after a patient’s death. Participates in IDG and ITC meetings to develop and revise the plan of care, ensuring psychosocial needs are addressed; reports updates on volunteer and spiritual care for Hospice patients. Assesses bereavement needs and develops a care plan within six weeks of a patient’s death; assists the Bereavement Coordinator with assessment, supervision, counseling, and record keeping per Hospice policy. Performs performance improvement studies as requested. Maintains knowledge of regulations pertinent to responsibilities. Stays abreast of overall patient care and enhances social work skills through participation in staff meetings, patient care conferences, audits, utilization reviews, quality improvement activities, in‑services, interdisciplinary team conferences, mandatory in‑services, CEU requirements, and ongoing professional development. Qualifications Has a baccalaureate degree in Social Work from an accredited school of social work. Will consider a MSW with a Master’s Degree. One year of social work experience in a health care setting preferred. Maintain current CPR certification (BCLS). 3HC is an equal opportunity employer.
