Medical Assistant
Habitat Health empowers older adults to experience more good days in their homes and communities. Through the Program of All-Inclusive Care for the Elderly (PACE), we provide comprehensive medical care along with support for daily needs such as meals, transportation, and in-home assistance. The Medical Assistant supports the delivery of high-quality, participant-centered care by providing clinical and administrative support within the PACE center. This role partners closely with providers and the Interdisciplinary Team (IDT) to support participant flow, perform routine clinical procedures within scope of practice, maintain accurate documentation, and communicate changes in participant status. Through reliable follow-through, attention to detail, and a collaborative approach, the Medical Assistant contributes to a safe, efficient, and welcoming care experience for participants.
Core Responsibilities & Expectations for the Role
- Supports efficient participant flow by completing intake, rooming participants, obtaining vital signs and screenings, fulfilling provider orders, and preparing participants for examinations and treatments.
- Performs laboratory testing, phlebotomy, EKGs, immunizations, injections, continuous glucose monitoring and other routine clinical procedures as directed by the healthcare provider and within scope of practice.
- Partners with providers and the Interdisciplinary Team (IDT) to communicate relevant observations, participant needs, and changes in status that support coordinated, participant-centered care.
- Coordinates follow-up visits, external appointments, annual and biannual assessments, and other clinical activities in collaboration with the care team to support continuity of care.
- Serves as a first point of contact for participant questions and care-related needs, responding through phone calls, electronic inboxes, and other communication channels, and appropriately addressing or escalating requests to support timely, coordinated care.
- Maintains accurate, timely, and complete participant intake and clinical documentation within the Electronic Medical Record (EMR) and other organizational systems.
- Supports clinical operations through referral tracking, medical record management and transmission, authorizations, and other administrative activities that facilitate timely participant care.
- Maintains clinic readiness by supporting routine clinic logs, exam room preparation and stocking, medical equipment readiness, and inventory management in partnership with center leadership.
- Provides a welcoming, respectful, and participant-centered experience while supporting the unique needs of older adults and frail elderly participants.
- Contributes to continuous improvement by proactively identifying opportunities to strengthen clinical workflows, participant experience, team collaboration, and quality of care.
- Performs other related duties as assigned.
Required Qualifications:
- High school diploma or equivalent; completion of an accredited Medical Assistant program.
- Current Medical Assistant certification or credential as required by applicable state law and organizational requirements.
- Current phlebotomy certification.
- Demonstrated ability to perform routine clinical procedures within scope of practice, including participant intake, vital signs, screenings, specimen collection, and other clinical support activities.
- Strong communication and collaboration skills, with the ability to effectively engage participants, caregivers, providers, and interdisciplinary team members to support coordinated, participant-centered care.
- Strong organizational skills with the ability to manage multiple priorities, maintain attention to detail, and ensure timely follow-through in a fast-paced clinical environment.
- Experience using Electronic Medical Records (EMRs) and the ability to quickly learn clinical documentation and other organizational systems.
- Active CPR/BLS certification or ability to obtain certification within 30 days of hire.
- Meets all applicable participant-facing health requirements, including required communicable disease screening, TB clearance, and immunizations.

Nice to have:
- Experience working within PACE, geriatrics, primary care, or other integrated or value-based healthcare environments.
- Bilingual: Spanish, Mandarin, or Cantonese.
- Minimum of two (2) years of experience working with older adults, frail elderly populations, or in a healthcare setting serving individuals with complex care needs.

- Remain stationary and/or move about for extended periods, consistent with job duties.
- Operate standard office equipment and/or clinical tools (e.g., computers, phones, medical devices, documentation systems).
- Travel between work sites or community locations as needed.
- Follow safety procedures, infection control protocols, and use personal protective equipment (PPE) when required.
- Perform tasks that may involve bending, reaching, lifting, or assisting participants, depending on role responsibilities.
- Lift, carry, push, or pull supplies, equipment, or materials typically up to 2550 pounds, with assistance or mechanical aids, depending on job duties and role responsibilities.
Compensation:
The anticipated salary range for this position is $26.00 - $30.00/hour. This position is also eligible for a target annual bonus of 5% and may be eligible for additional incentive compensation, as applicable. The final compensation offered will be determined based on factors including, but not limited to, the candidate's qualifications, skills, experience, education, certifications, geographical location, and business needs. In addition, this position is eligible for a comprehensive benefits package, including medical, dental, and vision insurance; paid parental leave, short- and long-term disability coverage; life insurance; flexible spending accounts; a 401(k) savings plan that includes a match; paid time off; and company paid holidays.
