Key Responsibilities

• Provide direct physical therapy services to residents in a skilled nursing facility setting
• Evaluate patients and develop individualized treatment plans to address functional deficits
• Implement therapeutic interventions to improve mobility, strength, and overall functional independence
• Document patient progress and maintain accurate clinical records in accordance with facility and regulatory standards
• Collaborate with interdisciplinary care team members to ensure coordinated patient care

Required Qualifications

• Active Physical Therapist (PT) license in the state of Massachusetts
• Graduate of an accredited Physical Therapy program
• Experience working in a skilled nursing facility (SNF) setting preferred
• Current CPR/BLS certification

Preferred Experience / Skills

• Prior experience in a SNF or long-term care environment
• Familiarity with Medicare documentation and compliance requirements
