About the Position

Job Title: Registered Nurse Profession: Nursing Specialty: Hospice Duration: 8 weeks Shift: Day 5x8-Hour Hours per Shift: 08:00 - 17:00 Experience: Minimum 1 full year Adult Home Health Experience or 1 full year Hospice experience License: Must have license in hand Certifications: CPR, BLS Must-Have: Proficient in Oasis and HomeCare HomeBase EMR within the last 6 months Description: Provides home nursing care according to physician orders based on agency policies and procedures. Coordinates and directs the provision of care through the competent application of the nursing process. Responsible to the Clinical Manager. Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses. Performs home visits and attends periodic office meetings. Must have reliable transportation, a current driver's license, and agency-required liability insurance. The average number of patients per day is 5-10. Weekly productivity expectations are 30 points, with each type of visit and activity assigned different point values. Types of cases include Start of Care, Wound Care, and Follow-up Visits. Traveling up to 75 miles from the home clinic is required. Mileage is reimbursed at the IRS standard rate. All professionals must be proficient with HomeCare HomeBase and Oasis. Documentation must be completed within 24 hours of the appointment. All travelers must accept every patient on their schedule. Declining patients may result in cancellation. There are no guaranteed hours, and productivity must meet expectations. Opportunities to float to other clinics may be available for additional hours if needed. Local candidates are accepted at the same rate.
