Health Homes Care Coordinator - Child & Family Services
SMH CASE MANAGEMENT
St. Mary’s Healthcare System for Children in Uniondale, NY seeks a Health Homes Care Coordinator to coordinate services for children with chronic conditions, build relationships with families, and navigate systems to implement a comprehensive plan of care. You will liaise with physicians, nurses and social workers to ensure timely, coordinated care.
