Infusion Pharmacist - Per Diem

Job Summary:Responsible for providing comprehensive pharmaceutical care to cancer and palliative care patients. Responsible for providing clinical pharmacy leadership within the Hematology/Oncology specialty area through the continuum of care.Essential Duties and ResponsibilitiesSupports and promotes the mission and values of Covenant Health Ministry.Assists with verification of treatment plan medication orders for dosing, interactions, compatibility, allergies, contraindications, and overall appropriateness of therapyCompounds and/or dispenses chemotherapeutic and infusion medications in accordance with all NH state and federal rules and laws.Consults with Physicians and Nursing staff in the Infusion CenterWorks with external providers to coordinate care for those with orders for outpatient infusionsProvides updated drug information to providers, staff, and patientsOptimizes safety and cost effectiveness of treatment plans through collaboration with the primary treatment teamRoutinely reviews biosimilar utilization and compares to current insurance coverages, GPO costs, and CMS reimbursement tables to evaluate cost effectiveness of treatments.Along with the Director of Pharmacy, responsible for the receiving, handling, and record keeping of investigational medicationsParticipates in the education and training of all pharmacists who will provide services and support the infusion centerParticipates in the management of the distribution, inventory, accountability of controlled substances in accordance with all NH state and federal rules and lawsSupervises Pharmacy TechniciansPerforms any task required to provide safe and efficient medication therapy to patients.Participates in performance improvement projects for the inpatient pharmacy and infusion centerConducts medication utilization reviews for infusion and/or chemotherapy agentsAttends and participates in all relevant hospital in-service programs, committees and required events.Participates in Pharmacy training programs and staff meetingsParticipates in appropriate role in hospital disasters, fire drills, and other emergency situations.Performs additional tasks as directed by the Director of PharmacyExercises care in the operation and use of hospital equipment and reference materials. Performs routine cleaning and preventative maintenance to ensure continuing functions of equipment.
