Schedule:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM

Role Expectations:

• Complete 30 points per week (35 patient visits daily)
• Conduct daily travel within assigned territory (mileage reimbursement provided)

Key Responsibilities:

• Conduct comprehensive OASIS assessments and develop individualized patient care plans
• Provide skilled nursing services, including wound care, IV therapy, medication management, and chronic disease interventions
• Monitor and evaluate clinical progress, promptly communicating changes to the interdisciplinary team and physicians
• Educate patients and families on disease processes, medications, treatment plans, and preventive care strategies
• Maintain thorough, timely documentation using Home Care Home Base (HCHB)
• Collaborate effectively with therapists, social workers, and home health aides to facilitate comprehensive patient care
• Participate in case conferences, staff meetings, and ongoing education
• Ensure adherence to regulatory and agency standards for documentation and care delivery

Qualifications:

• Active RN license in good standing for the state of practice
• At least 1 year of recent field-based Home Health nursing experience
• Proficiency with Home Care Home Base (HCHB) and OASIS documentation is essential
• Reliable transportation, valid drivers license, and current auto insurance
