Shift: Mon - Fri 0830-1700, weekend rotation every 6 weeks

Daily Caseload: 8-10 patients per therapist

Special Requirements:

• Certifications: BLS, State Licensed
• EPIC experience required
• Acute Care work setting experience required
• CVA rehabilitation experience required
• Adaptive equipment experience and assessment required
• Hip Fracture/total hip replacement, Knew injuries/total knee replacement exp required
• Lifting and transfer, patient/family education, safety/Injury prevention, telephone calls required

Billing/Documentation: assessment/discharge/evaluation/plan of care/reassessments required

Cognitive assessment exp preferred

Multiple Sclerosis and Parkinsons disease experience preferred
