Physical Therapist

Mount Laurel, NJ 08054 – Full-time, Home Health Physical Therapist position covering Burlington County, NJ. Overview As a home care Physical Therapist, you will be an integral member of a multidisciplinary health care team that provides rehabilitative care and skilled nursing to clients, helping them receive medical care at home. Responsibilities Perform diagnostic tests and measurements (mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, safety assessments). Implement integumentary protective interventions to prevent wound formation and promote wound repair using evidence-based dressings and topical agents. Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions. Continuously assess and revise the Physical Therapy care plan and collaborate with nursing in the multidisciplinary care plan as appropriate. Educate and instruct clients, families, or other representatives in rehabilitative care and activities necessary for client health, safety, and independent living. Accurately document observations, interventions, and evaluations using a state-of-the-art touch pad tablet. Qualifications NJ Physical Therapist – PT license (current). Graduation from an APA-approved program or equivalent, as shown by transcript or diploma. Ability to work independently and manage time effectively.
