• Weekly Estimated Pay: $2300-$2730
• Schedule: Day shift, 5x8-hour schedule
• Hours per Week: 40
• Patient Caseload: Approximately 20-23 patients
• Floating: May be expected to float to a nearby Laguna facility
• Weekend Requirement: Variable 5-day workweek based on department operations across 7 days per week; days off may vary

Job Requirements

• Minimum 1 year of acute care social work experience required
• Master of Social Work (MSW) required
• Previous travel social worker experience required
• Pending California social work license accepted, but active licensure must be in place by start date if applicable
• Strong communication, care coordination, and patient advocacy skills
• Ability to manage a dynamic caseload in a hospital setting

Responsibilities

• Provide psychosocial assessments for patients and families in the acute care setting
• Develop and coordinate discharge plans to support safe transitions of care
• Collaborate with interdisciplinary care teams to address patient needs and barriers to care
• Assist with resource planning, referrals, and community support services
• Advocate for patients and families while promoting quality outcomes
• Maintain timely and accurate documentation in accordance with facility standards
• Support department operations, including weekend coverage as needed
