Pine Park Health is looking for a Full-Time Nurse Practitioner / Physician Assistant responsible for delivering care in our Arizona Central Phoenix/Tempe/South Scottsdale market. Benefits Tailored for You and Your Family: Generous paid time off of 265 hours annually between vacation, sick, and paid holidays We offer flexibility, including work-from-home learning days on Fridays Comprehensive medical, vision, and dental insurance for you and your dependents Licensure paid at 100% Flexible spending accounts for health and dependent care expenses 401(k) retirement plan to help secure your financial future Equity 6 weeks of paid parental leave for growing families Monthly wellness allowance to support your physical and mental health Company Cell What you'll do: Willingness to provide onsite clinical care across the Central Phoenix/Tempe/South Scottsdale region Provide onsite primary care services to older adults living in Senior Living Communities Develop and execute detailed person-centered care plans on all primary care patients Work with Medical Director to identify and develop policies to address issues that arise which affect day-to-day practice, culture or clinical guidelines, including for emerging disease(s) Provide monthly Chronic Care Management (CCM) visits and care; review and sign off on CCM documentation Identify ways of working with senior living management and care teams to better carry out Pine Park care plans and improve patient outcomes Qualifications: Comfortable with a fast-paced and changing environment Desire to work with complex care patients and support the building of a holistic care model Strong orientation towards collaboration and working closely with a multidisciplinary team; values input of all members of the clinical team Strong written and verbal communicator Ability to work with tech-enabled care delivery tools (e.g., project management, 'checklist' management, remote monitoring, etc.) used together to improve patient care and team function This role involves driving between partner communities as needed Why You’ll Love Us: We’re a smart, heart-centered, low-ego group motivated to make waves in the lives of seniors and the healthcare industry. The final compensation package for each successful candidate will depend on several job-related factors unique to each candidate. These factors may include but are not limited to education, training, skill set, years and depth of experience, certifications and licensure, business needs, internal peer equity, and alignment with geographic and market data.
