Evaluate patients in both inpatient and outpatient settings, crafting personalized treatment plans. Administer targeted physical therapy interventions emphasizing mobility, strength, balance, and function. Collaborate with a multidisciplinary team including nurses, physicians, case managers, and other rehabilitation professionals to optimize patient outcomes. Educate patients and their families on therapeutic objectives, discharge plans, and at-home exercises. Consistently document evaluations, treatments, and patient progress according to institutional and regulatory protocols. Actively participate in care team discussions and interdisciplinary meetings to ensure seamless and coordinated patient care.

Doctorate in Physical Therapy (DPT) or equivalent from an accredited academic program. Valid state licensure as a Physical Therapist. CPR certification is preferred. Additional specialized certifications (neurological, geriatric, etc.) are welcomed but not mandatory.

Previous experience in the field is strongly preferred. Capability to treat patients with multifaceted medical and mobility challenges. Excellent communication and teamwork abilities. Proficiency with digital charting and clinical documentation tools.
