Fertility Care Navigator - Patient Journey Guide

Spring Fertility in Danville, CA is seeking a Patient Navigator to partner with our Care Team and help patients navigate their fertility journey. You will work with nurses to provide information and ensure a seamless transition to the nursing team for cycle management. Day-to-day duties include creating journey maps, tracking tests, building trust with patients, and explaining medical processes in an understandable way.
