What minimum requirements you'll need Licensure / Certification / Registration: * Social Worker credentialed from the Texas State Board of Social Worker Examiners obtained prior to hire date or job transfer date required. * Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred. Education: * Bachelor's degree in Social Work required. Work Experience: * 3 years of experience preferred. What additional preferences we're seeking * Clinical Case Management Experience: A proven track record of coordinating complex discharges and care transitions within an acute care or clinical setting * Active State Licensure: Current and unrestricted license in Social Work (LMSW/LCSW) as required by state regulations.
