Care Coach - Transition Coordinator LPN

Role Summary Telephonic Care Coach 1, working in a telephonic environment, evaluates members’ needs and requirements to help achieve and/or maintain an optimal wellness state. The coach guides members and families toward appropriate resources and facilitates interaction with services that support members’ health and well‑being. Work assignments range from straightforward to moderately complex. Responsibilities The Telephonic Care Coach employs a variety of strategies, approaches, and techniques to support members’ optimal wellness. Key duties include: Coordinating services and resources to address members’ needs. Identifying and resolving barriers to effective care. Ensuring members progress toward desired outcomes through continuous monitoring, assessment, data analysis, and conversation. Applying professional concepts, standards, regulations, strategies, and operating standards within the work area. Making decisions regarding work approach, priorities, and follow‑up, while adhering to documented procedures and professional standards. Required Qualifications Must reside in the state of Oklahoma. Unrestricted Oklahoma Licensed Practical Nurse license. At least 1 year of related experience. Knowledge of Microsoft Office Word and Excel. Exceptional verbal, written, and interpersonal communication skills. Proficient with various electronic information applications and software programs. Passion for contributing to a consumer‑focused organization committed to continuous improvement. Preferred Qualifications Prior experience with Medicare & Medicaid recipients. Experience in electronic case note documentation across multiple computer applications/systems. Experience with health promotion, coaching, and wellness initiatives. Knowledge of community health and social service agencies and additional community resources. Travel Occasional travel to Humana offices for training or meetings may be required. Scheduled Weekly Hours 40 hours per week. Pay Range $53,700 - $72,600 per year (starting base pay; may vary based on location and qualifications). Benefits Health benefits effective day 1.
