Case Manager: Flexible, On-Call Family Support
Southwest Key Programs
Paycom - ATS in New York is seeking a Case Manager responsible for completing and submitting assessments and service plans to compile a comprehensive case file across SWK's web-based program. The role requires maintaining organized schedules and may involve evenings and weekends.
You will conduct initial intake with clients and families, ensure timely documentation within 48 hours, maintain files, provide crisis intervention, and perform home visits as scheduled.
