Registered Nurse - Home Health

Job Description

Job Summary:

Seeking a dedicated Registered Nurse to perform in-home assessments and evaluations for a home health program. The role involves conducting hybrid telehealth and in-field assessments to support patient care planning and enrollment in managed care health plans.

Responsibilities:

• Conduct in-home pre-evaluation assessments to determine health status, strengths, care needs, and patient preferences.
• Review medical documentation and health referral forms to inform evaluation processes.
• Enter and transmit evaluation data accurately into electronic systems.
• Document concerns, conflicting information, and issues observed during assessments.
• Communicate effectively with consumers regarding healthcare options, support services, and waiver possibilities.
• Collaborate with interdisciplinary care teams to ensure coordinated and continuous patient care.
• Gather quality review data and support outcome measurement efforts.
• Maintain comprehensive knowledge of community resources, payer requirements, and network services.
• Ensure adherence to all established standards and complete assigned duties as directed by management.

Qualifications:

• Current RN licensure in New York State.
• Minimum of two years recent experience conducting comprehensive assessments in a home health setting.
• Proven ability to work independently in a fast-paced, deadline-driven environment.
• Strong organizational, interpersonal, and communication skills.
• Experience with electronic health records and evaluation tools.
• Knowledge of state and agency healthcare regulations.
