Licensed Practical Nurse (LPN) – Home Health

Salary: $35–$45 per hour Bonuses: Referral Bonus + Performance Bonus

Responsibilities:

Administer medications and treatments as prescribed by healthcare providers

Monitor and document patient vital signs and overall health status

Assist with daily living activities such as bathing, grooming, and meal preparation

Educate patients and families on managing health conditions and care plans

Coordinate with RNs, therapists, physicians, and other healthcare professionals

Maintain accurate and timely documentation using EHR systems

Provide emotional support and companionship

Follow all safety and health regulations in home care settings

Requirements:

Active and valid LPN license in Massachusetts

Minimum 1 year of experience in home health or related healthcare setting

Current BLS certification

Reliable transportation and willingness to travel to patient homes

Proficiency in electronic health records (EHR)

Strong communication and interpersonal skills

Compassion and commitment to quality patient care
