Key Features of the Role Deliver continuum‑based care integrating primary, psychiatric, palliative, and home‑based services across homes, assisted living, hospice, and nursing facilities. Collaboration with multidisciplinary teams, including psychiatric, palliative, and neurological specialists. Autonomy paired with structured onboarding, coaching, and clear playbooks. Dementia‑informed care leadership, building expertise in cognitive health initiatives. Focus on measurable outcomes and patient‑centered care metrics. Your Responsibilities Deliver personalized, high‑quality care to seniors in homes, assisted living, hospice, and nursing facilities. Provide dementia‑informed, relationship‑based care across the continuum. Conduct comprehensive assessments and develop tailored care plans. Manage chronic conditions, performing care with skill and empathy for patients and families. Collaborate with cross‑functional partners (palliative, psychiatry, neurology) to deliver coordinated, whole‑person care. Build trusted relationships with residents, caregivers, and facility partners to enhance engagement and referrals. Maintain consistency in documentation, follow‑through, and patient communication.
