Grane Home Health & HospiceOffice Location: 1200 Camp Hill Bypass, Suite 205 Camp Hill, PA 17011Coverage area: Traveling throughout the territory to complete admissions is requiredSchedule: Full Time Monday-Friday 11AM-7PMResponsibilitiesComplete initial and ongoing comprehensive assessments of patients and familiesInitiate patient plan of care and reevaluate as necessaryUtilize health assessment data to determine nursing diagnosisAdminister medication and treatment as prescribed by the physician in the plan of careCounsel the patient and family in meeting nursing and related needsComplete and maintain accurate clinical notesCommunicate with the physician and participate in interdisciplinary group meetings to meet the medical and emotional needs of the patient and family/caregiversEnsure compliance with all state and federal legal and regulatory requirementsAdditional responsibilities as assignedQualificationsGraduate of an accredited nursing school with current licensure to practice in state of operationOne year nursing experience required, three or more years preferredExperience in hospice or a similar setting preferredKnowledge of the hospice philosophy of careCommitment to clinical and documentation excellenceAbout Our Line Of BusinessAt Grane Home Health & Hospice, our primary ambition is to bring first-class care to the warmest and most serene environment possible: home.
