Family Practice/Primary Care Nurse Practitioner

Conviva Senior Primary Care is seeking an experience clinician for our Float Nurse Practitioner/Physician Assistant position in Pinellas County, Florida mainly between two locations.

As a clinician with Conviva, you ll have the opportunity take a hands-on approach to addressing patient needs inside the exam room, as well as looking beyond to understand how social determinates of health are impacting health outcomes. We have a unique care model that sets us apart for patients and providers. We are experts in senior-focused primary care with a multidisciplinary approach.

A day in the life of a Conviva provider:

• See and average of 15 patients per day. Opportunity to spend more time with patients
• Thrive in a value-base care model and improve patient s quality of life
• See a variety in your caseload, from preventative care to chronic conditions
• Spend 100% of your time clinically focused on direct patient care, inclusive of patient facing time and general administrative time (charting, meetings, etc.) as it relates to direct patient care

Required:

• Active, unrestricted NP/PA license in the appropriate state
• Current certification as an NP or PA
• Board certification
• Current DEA license or ability to obtain a DEA license
• You will be required to be screened for Tuberculosis.
• Two years' or more experience working at an advanced practice provider seeing patients 65+ age group and/or seeing patients with chronic medical conditions

Preferred Qualifications:

• Familiar with value-based care model
• Outpatient senior care experience
• Familiar with HEDIS measures and Medicare Advantage
