Travel Physical Therapist (PT) – Home Health

Location: Marysville, CA
Service Area: Grass Valley, Yuba County, Butte County, and Sutter County
Shift: Days (08:00 AM - 04:30 PM)
Schedule: Monday – Friday

Responsibilities

• Evaluate patients and develop individualized treatment plans to improve mobility, function, and independence.
• Provide skilled physical therapy services in patients' homes.
• Assess patient progress and modify treatment plans as necessary.
• Educate patients and caregivers on therapeutic exercises, mobility techniques, and injury prevention.
• Complete accurate and timely clinical documentation.
• Collaborate with physicians, nurses, and interdisciplinary healthcare teams to coordinate patient care.
• Maintain compliance with home health regulations and agency policies.

Required Qualifications

• Active Physical Therapist License required.
• Current AHA BLS Certification required.
• Must be at least 21 years of age.
• Doctor of Physical Therapy (DPT) degree required for candidates who graduated in 2016 or later.
• Valid driver's license and reliable transportation required.

Preferred Qualifications

• Minimum 1 year of Home Health Physical Therapy experience preferred.
• New graduates are welcome to apply and will receive training.
