Ambulatory Cardiology Pharmacist — Pulmonary Hypertension

OhioHealth is seeking a pharmacist to join the Heart Disease Management team. This pharmacist role focuses on pulmonary hypertension and other heart conditions with ambulatory care emphasis and limited inpatient transitions of care. You will collaborate with an interdisciplinary team to provide evidence-based therapy management and patient-centered education.
