Good Samaritan Home Health is seeking a compassionate and skilled Registered Nurse (RN) Case Manager to join our dedicated Home Health team. In this role, you'll provide high-quality, patient-centered care to individuals in the comfort of their homes, while coordinating services to ensure patients and families receive the support they need.

What You'll Do

• Provide skilled nursing care, assessments, education, and support to patients and families.
• Serve as the primary case manager, coordinating care with physicians, therapists, aides, and other team members.
• Complete accurate and timely OASIS documentation, admissions, recertifications, and discharges.
• Monitor patient conditions, update care plans, and communicate changes to the interdisciplinary team.
• Educate patients and caregivers on disease management, medications, and strategies to promote independence.

Qualifications

• Minimum 1 year of nursing experience (home health or hospice preferred).
• Medicare/OASIS documentation experience a plus.
• Current CPR/BLS certification (or ability to obtain within 30 days of hire).
• Strong clinical assessment, communication, and critical thinking skills.
• Valid driver's license, automobile insurance, and reliable transportation.

Shift Details: Flexible schedules are available (40hrs, 32hrs, 24hrs, or scheduled PRN)
